 I hereby authorize Documented Reference Check (DRC), as an independent, third party, to conduct a reference check on the below information. I also authorize DRC to subcontract this Application For Reference Check out to other affiliates. The resulting document will become an official record kept in the normal course of business.

I understand I have a responsibility to protect my own interests, and DRC is a company qualified to conduct reference checks, but must refer all legal questions to qualified attorneys.

I understand DRC has no vested interest in the outcome of the report. Further, I will allow DRC to proceed without interference, or special directives. I will allow a fair, voluntary response from the targets listed below. My signature indicates my acceptance of the terms on back of this form, and the above terms.

RELEASE FORM

WHAT WE DO: Documented Reference Check (DRC) is skilled at unbiased extraction of employment information from previous employers. DRC duplicates the same process a Personnel Manager will employ before hiring you. To protect your confidentiality and ours, the actual work is performed by other companies (Not DRC).
WHAT YOU SEND US: (1) Complete this Application for Reference Check. (2) Include a check or money order. (3) A mailed submission could include a resume. (4) A mailed submission could include a letter of explanation (no more than one page). Send order by regular mail. Mail requiring signatures is not possible, because we work in secured facilities. Allowing non-employees to enter premises would be a breech of security.
SUBSTITUTION POLICY: DRC reserves the right to substitute contacts if your listed contact is no longer employed at your former company, or they are unavailable for any reason. DRC reserves the right to substitute a telephone reference with mailed questionnaire if target does not respond, in our estimation, to our telephoned attempts.
RATES FOR SERVICE: Rate for a reference check is $87.95 U.S. dollars for each person you ask us to contact (One person = $87.95; two people = $175.90; three people = $263.85; etc. These rates are double when checking employers outside of the U.S.A.) Rates are subject to increase without prior notice.
GUARANTEES: DRC cannot guarantee the outcome of your order. We will duplicate the same process a Personnel Manager will employ before hiring you. This may result in defamatory statements, strong recommendations, refusal to comment, or other responses. You maintain the responsibility to protect your own interests. Such written reports are common and have, in the past, been used as a legal means of court procedure. Different attorneys and judges, however, will have differing opinions and guidelines, depending on circumstances. Therefore, DRC cannot and will not guarantee specific responses by employers, or the strategic value in any legal proceeding. DRC will document the conversation to the best of our ability and place this information in a written report. Also, as a condition of order placement, the liability of DRC for any error for which DRC may be held legally responsible, will not exceed the price paid for the order. DRC will not, in any event, be liable for loss of income, or profits, or any consequential damages.
TURNAROUND: THERE IS NO GUARANTEED TURNAROUND TIME! The average order, however, is completed in approximately 38 days. If your order is delayed, DRC has an Order Status line. Our Order Status line is (909) 629-6172. An e-Order Status is available at orderstatus@badreferences.com.
ATTORNEY REFERRALS: DRC is not a law firm, however, our site does link to another site that does list attorneys who specialize in employment law. These attorneys specialize in representing employees, rather than employers. This listing should not be considered a recommendation. In addition, these attorneys are not obligated to accept referrals and may decline at their discretion.
REFUNDS: Prior to the completion of an order, you may cancel an order for any reason and receive a complete refund. This refund is available for all targets that remain uncontacted. After contact has been made with a target (even prior to mailing completed report) there is no refund.
SPECIAL REQUESTS: It is our goal to be as effective as possible. It is also our goal to make this service as affordable as possible. We will be forced to ignore special directives, additional terms, or deadlines. Please allow our professional staff to proceed unencumbered by unusual requests. DRC reserves the right to refuse, or cancel any order it deems unacceptable. We cannot accept verbal revisions on orders. Please make all revisions in writing.
ADDITIONAL SERVICES: (beyond a reference check): DRC never invoices. All services require payment in advance. So, don't worry about being billed for services you did not request. Services beyond reference checks are, (1) Negative Removal at $68.95. (2) Tele-consulting at $2.00 per minute. (3) e-Consulting at $2.00 per minute. Average e-mail response is approx. $35.00. (4) Guest-Speaking at $1,200 per presentation. (5) Video Testimony at $120 per hour. Average testimony is approximately 2 hours. A retainer of $1,200 is required, in advance. (6) Live Testimony at $120 per hour, plus expenses. A retainer of $1,200 per day is required, in advance, out of which DRC is authorized to satisfy any of it's billings. Renewal retainer deposits may be required for extended requests. Unused balances will be refunded. Again, these services are only available by special arrangements--in advance.
SUGGESTIONS: DRC suggests that all orders be placed in writing through regular mail. Clients that send sloppy, or incomplete orders usually regret this when the documents become part of a legal action. Please be accurate. Do not threaten, or begin legal proceedings prior to our report. Nothing will dry up conversations quicker than a threat. Never disclose our report, or its contents to anyone, except your attorney.
